Advice for high intensity users of clinical health services

Dudley Community and Voluntary Service (DCVS) commission us to deliver an advice caseworker service to people living in Dudley Borough who access their Integrated Plus’s High Intensity User Service.
This service offers a robust way of reducing avoidable frequent user activity to 999, NHS 111, A&E and hospital admissions freeing up front health line resources to focus on more patients and reduce costs to the health service.
Citizens Advice Dudley and Wolverhampton deliver advice to some of the most vulnerable people within the community to help them flourish and find purpose in their lives.

How can this service be accessed?

Our caseworker service can only be accessed via a referral from the High Intensity Team within Integrated Plus.
